Men liable for military service from 18 to 60 years old are subject to conscription during mobilization.. Soldiers who have already participated in hostilities receive a corresponding certificate. What documents are needed to restore the UBD certificate.
To recover a lost or damaged ID, the following documents are required:.
an application for the replacement of a damaged / lost certificate, specifying the reasons for the loss (in the application it is necessary to indicate the phone number);
in case of loss of the certificate (if possible), it is necessary to submit an advertisement to the newspaper about the loss of the document, indicating the surname, name, patronymic, which are indicated in it;
in case of damage to the certificate - bring the damaged certificate;
a copy of the lost certificate (if any) or an indication of the number, series of the certificate and date of issue;
copies of passport pages (1,2 and 11,12), copies of a certificate of assignment of an identification number;
photo size 3 * 4 - 2 pcs.
