In Ukraine, parents can register the birth of a child and obtain a birth certificate using the єMalyatko service. The Diya portal told how to get services in the eMalyatko service offline and in what cases it is impossible to do this.

How to get a service in eMalyatko.

To receive the service in offline mode, you need to contact the administrator of the eMalyatko service directly at the maternity hospital, ASC or the registry office at the place of birth of the child or the place of your registration.

You should also prepare the original passports of both parents. Birth certificate number required. If the maternity hospital does not issue medical certificates, you will be issued a medical birth certificate (Form 103/o). The following information will also be required when completing the application:.

about the identification numbers of both parents (if any);

from the marriage certificate (date, place and authority that concluded the marriage);

UZR of at least one of the parents - to order a service for assigning a UZR to a child;

bank account number in IBAN format – for ordering the service of prescribing assistance at birth.

When filling out the application, the presence of both parents is required if the father and mother of the child are not officially married. In the event that the father and mother of the child have different surnames, the presence of both parents or a notarized power of attorney and the consent of the absent parent to give the child a surname is required..

If the father and mother of the child are registered at different addresses, the presence of both parents or a notarized power of attorney and the consent of the absent parent to register the born child with one of the parents is required.

In what cases it is impossible to receive the service offline.

The service is not available if:.

the child already has a birth certificate;

the child was born outside the hospital;

the child was not born in Ukraine;

one of the child's parents is a citizen of another country;

the marriage between father and mother took place in another country.
