What to do if you lose your UBD ID.
The department explained that in Ukraine, the electronic ID of a veteran in “DII” has identical legal force as the paper one, confirming his status. A person can also restore a lost paper UBD ID.
To restore your UBD ID, you must contact:.
Department of Veterans Affairs;
Structural unit for veteran policy issues. They work under OVA, district state administrations, city executive committees;
Administrative Services Center (ASC).
You also need to submit an application and documents in person/through a legal representative or send by mail (registered mail).
What documents are needed to restore a UBD ID?.
To restore/replace the UBD certificate, which was issued by the Ministry of Veterans, you should contact the address Khreshchatyk street, no.. 34, g. Kyiv, 01001 or through the Central Administrative Service (only for Defenders and Defenders from among volunteers who received the status of UBD according to the decision of the interdepartmental commission formed by the Ministry of Veterans).
You should also submit an application in any form, which indicates your full name, postal address and email address, telephone number, method of obtaining a combat participant’s certificate (at the place where the certificate was issued, at the Central Administrative Office (full name and location) and attached:.
a copy of a passport of a citizen of Ukraine or a temporary certificate of a citizen of Ukraine - for citizens of Ukraine;
a copy of a foreigner's passport document or stateless identity document, permanent residence permit, residence permit, refugee certificate, certificate of a person in need of additional protection, or other document confirming the legality of the stay of a foreigner or stateless person on the territory of Ukraine, except for a certificate of application for protection in Ukraine and a certificate of application for recognition as a stateless person - for foreigners and stateless persons;
copy of the registration number of the taxpayer's account card;
a copy of the identification document of the legal representative or authorized person, and a copy of the document that grants the authority to the legal representative or authorized person to represent the applicant, drawn up in accordance with the requirements of the law - in the event of an application from the legal representative or authorized person;
photo size 3*4 centimeters;
extract from the Unified State Register of War Veterans (it can be obtained from the Central Administrative Service);
certificate that requires replacement;
documents on changes in personal data (if available).
The Ministry of Veterans noted that the restoration or replacement of UBD certificates for military personnel, as well as privates and commanding officers, is carried out by the security and defense sector authorities in which they serve or served.
