How to find a doctor with whom you can sign a declaration.

If you want to find a family doctor, therapist or pediatrician in a specific institution, select it in the table below the map or enter the name in the filter. Below you will see a list of outpatient clinics and their contacts.. You can also click on the desired clinic on the map or in the table below.

To the right of the map, a list of all primary doctors who accept in the institution that you have chosen will be displayed.

You can submit a declaration to a doctor, opposite the name of which there is an up arrow in the dashboard.

If the dashboard shows that in your locality all primary doctors have exhausted the limit, then call this institution by phone to clarify whether it is possible to file a declaration.

“If it is not possible to choose a doctor, the patient has the right to apply to the local authorities, which are the owners of health care institutions, or to the local health department. They should explain how the patient can exercise his right to choose a doctor,” the National Health Service noted..
