Dr. Komarovsky on the medreform: We go naked and barefoot on a slippery road

The monologue of the pediatrician, the author of several popular books for parents, TV host Yevgeny Komarovsky, was published in the Novoye Vremya journal for March 22, 2018. I have a difficult attitude towards the current reform of Ukrainian health care. On the one hand, its steps are correct and long overdue, but not at all backed up legally, economically and informally. Everyone understands that the existing MHO team is temporary, like the figure and. the Minister. Everyone knows that there will be parliamentary elections, respectively, the new Cabinet of Ministers. And those who will come, who will they be? In politics, whatever you take, everyone is against the current head of the MOH. For her acting prime minister, but how many people around say that in the next election this team to power may not reach. Very many of the current judicious initiatives of the Ministry of Health can not be realized because of a shortage of funds, and the number of ill-wishers who want this team to steal something or something is wrong, it grows. I would very much like to have the authorities assure the next election that the reform of medicine is not retroactive. Suddenly, once again comes a tough experienced official who will restore the Soviet medicine and tell us what a unique. I'm very scared of all this.. Therefore, yes, we are moving in the right direction, but on a slippery road. We go naked and barefoot, and the chances to achieve the goal we have a little. In addition, the survivors and those who set out on a journey periodically question, but do they have to walk this way? The migration of professional medical personnel abroad is enormous. Since April, as planned by the reformers, we all have to sign declarations with their own doctors. However, it is unrealistic to cope with this during the time left before the parliamentary elections.. I tried to do this in my city of Kharkov. I know a doctor who I respect and see with my doctor. But in the current lists there is neither his polyclinic nor his surnames. That is, I can not do this. Why? I do not know. But if the system works in Kharkov, then in small towns it's even worse. The problems are not only in this. The MOH and the entire health system can not exist separately from the country, it lives according to its laws. For example, you are a patient who comes to me with pneumonia. And I say - it's pneumonia, it needs to be treated with this medicine. But I do not have money for this medicine, and if there is, then this medicine in the pharmacy is fake. And reform on health. Everyone says the right thing, but in the country there is no money for medicines and good doctors, there is no working law, strictly punishing for falsification of medicines. People in Ukraine often say: we want European medicine. So, first, they need to explain what it is - European medicine, to dispel illusions. In European medicine, it is inconceivable that at a high temperature of the child the doctor comes to your house on call. A whole doctor, yes even with a nurse! And you gave him 100 UAH and wondered why he was not happy. Every woman has earrings or a ring, but no one thinks that where in Belgium a personal jeweler does them personally for her. The mechanic does not come to your home to repair your car, he will invite you to a maintenance station. But we are used to the fact that the doctor comes to our home. We have lost the uniqueness of the profession of a doctor, therefore we are unlikely to be delighted with the "real European medicine". There are things I'm praying for in European medicine - this is a guarantee of the quality of drugs in pharmacies, the absence of the notorious "patience". No one will hurt a person if there is a possibility to avoid it. And you know: if fate turns away from you and you die, medicine guarantees the quality of death - you will not die in agony and on wet sheets. Both the patient and the doctor are people whom the European society respects. We have our own, Ukrainian national medicine with "fuflomycins" and oxolin ointment, which for some reason differs from the global world based on evidence and research. Ukrainian doctors rarely treat according to international protocols, they talk about the need for local. But local protocols are in most cases not needed. For example, there is a protocol for outpatient care in pediatrics. Parents came with complaints. The nature of complaints follows a specific survey protocol and specific actions based on its results. These protocols are universal and uniform for the whole world. All the world's pediatricians at a child's high temperature or atopic dermatitis react equally. Another thing, stationary medicine: heart attack, oncology, when we are simply compelled by our poverty to come up with local rules of assistance, because the state can not allocate funds for such a protocol. In 5% of cases, local protocols are important, taking into account the economic capabilities of the state, the availability of drugs from local producers, and other. In all other cases, the actions of a doctor in Ukraine should not differ from the actions and prescriptions of a doctor at the other end of the world. Refuse first aid in its current form - a crime. There should be a family doctor who knows: if his patient with temperature or high pressure does not turn to him, but to the ambulance, he will not earn his money. It is necessary to grow normal paramedics, able to do everything possible to save lives in emergency cases. Only then can I remove the ambulance in its present form. This is an important topic, but see what a "boring". In a normal country, on the topic of medical reform, there should be a global political talk show once a week, which explains to people why medical protocols are important, what kind of emergency should be. But our interesting for the hundredth time to discuss what language we speak and who else to demolish the monument. Outbreak of measles Ukrainians really scared. But, and I talked about this, people start to change only when they see real deaths. Without coffins in yards, we do not believe in vaccination. But I do not want people to be controlled by fear. I want to be controlled by knowledge. We used to be frightened, and it's disgusting to work with people at this level. I am not satisfied with the vaccination situation in the country, the problems did not stop. Individual positions up to 40% of the population are not vaccinated. We talk a lot about children, and what percentage of adults are vaccinated against diphtheria? At best, 1%. We are still lucky that diphtheria rods with weak toxin circulate in Ukraine. But it is worth to come here to a man from Yemen, Bangladesh or Venezuela, where there is a real epidemic of diphtheria and spreads highly toxicogenic diphtheria bacillus, everything can change in a moment. It only comforts that if such an epidemic rips in Ukraine, all neighboring civilized countries will supply us with serum to protect themselves. The absence of collective immunity from such diseases is a national security problem. But in our country there is no body that deals with national security. The one who is, is busy with something quite different. Not vaccinated with a single. The complex of prevention and early monitoring of diseases does not work in the country. A family doctor should tell patients which examinations and procedures need to be completed at 30, 35, 40 years, so as not to get sick. Moreover, it should be advantageous for him to send you for early diagnosis. He is obliged to understand: if you are a hypertensive person, then you will start to visit him ten times a month for the same money. But they do not know how to work. Our medical universities have learned to produce doctors for this "national" medicine, where the health worker does not live at the expense of the ability to treat well, but at the expense of his patient. Conflict and. minister with the Kiev medical university has shown much. So what? Someone supported the minister? The old system immediately reacted to an attempt to interfere with it. Uliana Suprun is not clear to me. With all her good endeavors, she has a huge problem with communications. She somehow manages to set herself against 90% of the population. Why publicly talk about your attitude to the holiday of the eighth of March? In Ukraine, for the most part, it is different. If you are a minister, a public figure, work with society carefully. Not understanding you, not trusting you, it will not trust the reasonable and right that you do. I want to be an optimist, but nothing makes me happy. On the one hand, I see more and more publications from adequate doctors on Facebook against "fuflomycins", on the other - queues in pharmacies do not diminish.

The amount of falsification in pharmacies is also not reduced. Complete neglect of the law remains. Remains deficit, humiliation of doctors. I have a lot of things in Ukraine, and I will not leave this place. Although, who knows, maybe tomorrow shrews come and say: we to you, doctor ... I now help people who still stay here, train them to survive the time before the reform of current medicine. I dream that these innovations will ever come, and then I will not teach Mom, but family doctors. So let's fight.
