statement of consent to provide social services;

declaration of consent to receive them;

passports of the applicant and the person in need of social services (submitted when applying);

identification codes;

declaration of income and property status (filled out on the basis of certificates of income of each family member for the previous 6 months);

certificate to the inspection report by the medical and social expert commission (for persons with disabilities);

conclusion of the medical commission of a medical institution on the need to care for elderly citizens due to cognitive impairment (for patients with such a diagnosis);

conclusion of the medical commission of a medical institution on the need to care for terminally ill persons, if they are unable to take care of themselves and move independently due to a violation of body functions.



As for the amount of compensation, it is calculated as the difference between the subsistence minimum per person per month, established on January 1 of the current year (from January 1, 2023 - UAH 2,589), and the average monthly total income of a person providing social services (income is taken into account.

The average monthly income of a person providing social services is calculated by dividing the average monthly total income of his family by the number of family members. That is, the amount of payment depends on income and is calculated individually for each recipient..
