In what kind of health facilities can I sign a contract?.

You can sign a contract with your doctor at any primary health care facility where a family doctor, therapist or pediatrician accepts.

In this case, it is not necessary that this is only a state or community health institution. The provider of services under the health guarantee program can also be private clinics. At the same time, even there, the state-guaranteed package of services for the patient will remain free.

Therefore, specify in the selected clinic the relevant information on joining the program.

In addition, when going to a clinic, polyclinic or an outpatient clinic to sign a contract, do not forget to check with the registry beforehand if they have time to connect to the general system "Electronic Health".

Does it matter when choosing a clinic?.

From now on the registration absolutely does not matter. Registration can be in Lviv, while you live in Kiev on Kharkov, and you can sign the declaration on Teremki.

So, you have the right to choose a doctor not on the basis of your passport with a residence permit, but based on your own comfort and the degree of trust in the doctor.

In addition, the contract with the doctor does not expire and does not oblige the patient to be tied to one doctor.

If necessary (for example, because of moving), you can at any time sign a new contract with another doctor (the declaration with the previous one will be automatically canceled), and also apply for medical assistance to any medical institution of Ukraine.

Should the whole family sign a declaration with one doctor?.

No, this is a rumor. As already mentioned, you can sign the contract with primary care physicians - with family doctors, therapists and pediatricians.

That is, you can finish the contract about the medical care of the child with the pediatrician, "register the relationship" with the family doctor, and your other half to choose the therapist.

What do you need to sign a contract with your doctor?.

Preliminary record in the medical institution is not needed. Signing of contracts is carried out in the order of a live order for the duration of the working day or in the days and hours allocated for it (here everything is at the discretion of the administration of the medical institution).

First of all, you need to have a passport, TIN, birth certificate of the child (if the contract will be signed for him) and, preferably, a mobile phone.

Go to the medical institution and find out in the registry where exactly you can sign the contract.

All data in the database will be entered by the employee of the medical institution. While registering your data in the electronic system, you will receive a message with a confirmation code on your mobile. If the phone does not, your documents will scan and attach the copies to the declaration.

After completing the contract in electronic form, the registrar will print you a copy of the document - you must check the correctness of all content. Otherwise, the document will be invalid..

Then, after making corrections (if any), you will print two more copies of the contract. Once again, all a good re-check, but only then sign. One of these copies remains with you.

What is the deadline for signing the contract?.

Ministry of Health asserts that there is no time limit for signing contracts. That is, throw all the cases and run to the clinic for the declaration is not necessary.

So, you can sign a contract by consulting a doctor when you need one.

However, this situation may arise here: the doctor you need has already signed the number of declarations that are acceptable for him, and you will have to look for another specialist (given that doctors are not enough, good doctors can "snap" at times).

At the same time, it is necessary to sign the contract until 2020. Since the reform of the medical sphere is planned to be completed at this time, and then, according to the Law "On State Financial Guarantees of Medical Care of the Population," "the doctor will not be obliged to provide primary medical care to patients refusing to sign the declaration".
