Ultrasound: what's important to know

In terms of complex diagnostics, 21 patients (12.5%) with leaf-shaped tumors underwent ultrasound (data Г. Korzhenkova): 14 studies (66.6%) in benign leaf-like tumors; 5 (23.8%) - for the intermediate; 2 (9,6%) - with malignant leaf-like tumor; and also in 3 cases (5.6%) of primary breast sarcomas. Of these, 8 patients underwent a complex ultrasound study, which included, along with a real-time study in the B-mode, pulsed dopplerography and color Doppler coding of vascular structures.

During the study, the following signs were analyzed: the nature of the contours of the formation of the breast (clear, fuzzy); presence of the surrounding hyperechogenic rim; structure of education (homogeneous / heterogeneous); the presence of fluid cavities, small cystic inclusions, slit structures; the presence of acoustic effects (dorsal amplification of the signal, central and lateral acoustic shadow).

In the group of benign leaf-like tumors, out of 14 cases: hypoechoic formation - 13 (92.8%), isoechoic - 1 (7.1%); heterogeneous structure - 11 (78.5%), homogeneous - 3 (21.5%); with a clear contour - 14 (100%); with the presence of a hyperechogenic border - 10 cases (71.4%), in 4 patients (28.6%) the effect of a hyperechogenic border was not detected (small tumor size - up to 4 cm). In no case was the central acoustic shadow detected, in 10 cases (71.4%) there was a lateral shadow, and in 9 cases (64.2%) - dorsal signal amplification.

Fluids containing cavities were detected in 5 cases (35.7%) in the form of cracks and small cystic cavities up to 0.5 cm (the sizes of the formations exceeded 8 cm). At small sizes of tumors apparent fluid-containing cavities were not revealed, however, a characteristic heterogeneous structure, reminiscent of the head of cabbage on the cut, allowed to assume a leafy tumor.
